Ishikawa Diagram & FMEA Table

The page is intended to aid in Root Cause Failure Analysis, following a standard Ishikawa (fishbone) layout. Click the problem, category names, and cause labels to edit their text. The data table below is synchronized with the causes in the diagram. It uses the Failure Modes and Effects Analysis (FMEA) approach to assess and prioritize risks associated with each cause. This uses a Risk Priority Number (RPN) calculated from Severity, Occurrence, and Detection ratings as means of prioritization for each potential cause. As this page is meant to provide a lightweight tool, no data is currently stored between sessions. Navigating away or refreshing the page may cause data to be lost. This is a work in progress. PDF and Excel export options are provided at the bottom of the data table.

Each category starts with five causes. You can add or remove causes per category; RPN computed as Severity × Occurrence × Detection.
